Behavioral Support
This service is available for individuals age 21 and older and includes comprehensive assessment, the development of and revisions to a behavioral support plan that includes strategies to support the individual based upon assessment, and the provision of interventions and training to individuals, staff and caregivers. There are two levels of service that reflect the differing levels of provider qualifications and the needs of the individual.

Community Participation Support
This service provides opportunities and support for community inclusion and building interest in and developing skills and potential for competitive integrated employment. This service may be provided in community locations, community hubs, licensed adult training facilities, licensed Older Adult Daily Living Centers and licensed vocational facilities.

Companion
This service is intended to assist an individual who is age 18 or older to participate more meaningfully in home and community life. It is provided to individuals living in private homes for the purposes of providing supervision or assistance that is designed to ensure the individual’s health, safety and welfare or to perform activities of daily living for the individual.

In-Home Community Services
Services to help people gain, maintain, and improve skills that allow them to live and participate in their local community. Examples of habilitation include supporting someone to participate in a church activity, learning to cook safely, or learning to use public transportation independently.
